Drop the transcript heading and empty-state hint in the conversation view (#163) ...

The encounter view showed an uppercase "Transcript" / "Conversation record"
label and "Start the conversation — <bot> is on stage above." before any
message. Both repeat what the screen already shows, so they are removed.
